Our Prevention Plan CM™ (Care Management) program targets the population that has already presented or been diagnosed with a chronic and costly health condition, including conditions such as coronary heart disease, diabetes, stroke, obesity and others. In these cases, an established health protocol has usually already been prescribed and provided to the individual by his or her health care provider.  Studies have proven that the greatest challenge to successfully managing any chronic condition is adherence to these prescribed protocols. 

The Prevention Plan CM™ provides an intensive program that helps people with chronic illnesses adhere to the protocol, manage their health better to lower their risks, and improve their overall health. The customized approach includes coordinating medical care, referral to community resources, and educating and coaching each member to help them modify their lifestyle behaviors and patterns.  This product can also be purchased at a group or individual level.
