Besides quantifiable outcomes, program sponsors receive a private, independent service that provides a comprehensive set of monthly, quarterly and annual reports that document  improvement  and cost management opportunities.

Here’s just a sampling of recent outcomes from our care management programs:

• A1c testing of members with diabetes increased from 47 to 100 percent testing following six-month program intervention. Also improved foot exams from 12 to 65 percent. (Florida program)
   
  
• The premature birth rate in Duval County, Florida was 13 percent, but the “high risk” group managed by the care managers at U.S. Preventive Medicine^® was 11.3 percent.
   
  
• A 47 percent reduction in emergency room utilization expenses on a per member/per month basis was documented for members with schizophrenia. (Colorado program)
   
  
• At their initial assessments, 26 percent of Program participants with asthma had an action plan endorsed by their physician. Following at least six months of program participation, 93 percent reported having an asthma action plan. (New York program)
