The Prevention Plan™ provides physicians with a wealth of information about the health of their patients and allows them to dedicate more time to helping patients understand and address risks for future disease. We encourage our members to share the results of their personal Prevention Plan with their physician so this information can be incorporated into an ongoing care plan.
Our Nurse Advocates also play a key role in strengthening the doctor-patient relationship. Through on-going telephonic coaching, our nurses encourage members to seek out medical care for issues that have been identified in their plan. They also educate members about their health risks, offer effective behavior modification strategies to improve those risks, and continually reinforce the need for members to follow their doctor’s care plans.
